Medical Monofilament is happy to accommodate custom orders. If you want to place a custom order, please review our custom terms before downloading and sending the M3 Custom Order Terms form to info@medicalmonofilament.com.

The name and number of a contact person responsible for making decisions regarding custom orders must be provided.

3. Before a work order is issued, artwork must be approved and signed off by client and a 25% custom deposit or payment in full is required. |
4. The Art Department requires up to a ten (10) working day turnaround time before the custom paper is delivered to M3. |

5. Once the paper arrives at M3, the shipping date is determined by quantity ordered as well as ordered received prior to. |
6. Incremental shipments for large orders to one location will be made unless other arrangements are established. |

7. Shipping and handling charges will be added to the invoice unless M3 is provided with the customer’s preferred carrier and account number. |
8. Payment in full must be received by Medical Monofilament before the custom order is shipped. |

9. Custom monofilaments can be packaged in one of three ways: Monofilaments in Blister Packs, Monofilaments with Instruction Cards, or Monofilaments in Tyvek® Envelopes. |
10. Artwork will be accepted in the following file types: MAC file, Jpeg, Quark, Illustrator, Tiff, EPS, Freehand, PageMaker or Photoshop image. |

11. In order to comply with FDA guidelines of guaranteeing the 10-gram force range, Medical Monofilament has placed a one-year expiration date on all monofilaments. |
12. While our Open PO offers customers the ability to order in bulk and obtain the best possible pricing, it is also important given the product’s expiration date. |

13. Medical Monofilament is not responsible or liable for Art or Logos received for custom monofilaments and/or instruction cards that is copywrited, trademarked or registered. |
14. Medical Monofilament reserves the right to showcase custom artwork on our website and other materials unless otherwise specified by the customer. |
