1. Minimum order of 5,000 units
2. Artwork* must be approved and signed off by the client before a work order is issued.
3. Medical Monofilament will provide the first proof at no cost within 3 business days of art being submitted. Any changes/add-ons/revisions thereafter will require a new proof at an additional cost to the customer.
4. A twenty five(25%) percent non-refundable deposit is required on all custom orders.
5. The name and number of a contact person responsible for making decisions regarding custom orders must be provided.
6. Once Medical Monofilament receives the custom order deposit, as well as the final sign off on the artwork, there is a ten (10) working day turnaround time before production can begin.
7. Incremental shipments for large orders to one location will be made unless other arrangements are established.
8. Shipping and handling charges will be added to the invoice unless we are provided with the customer's preferred carrier and account number.
9. Terms: Net 30
10. Custom quotes are valid for 30 days.

MAC file, Jpeg, Quark, Illustrator, Tiff, EPS, Freehand, Photoshop

Monofilaments are packaged in two (2 ways). Handles are complete with filament attached.
