Monofilaments are guaranteed for single usage only. Monofilaments should be stored in dry environment at room temperature. Medical Monofilament does not guarantee components or products that have been broken down and repackaged.

Medical Monofilament warrants its monofilament products to the original end user against defects in workmanship or materials under normal use in accordance with the instructions provided above.  THE ABOVE EXPRESS WARRANTY IS IN LIEU OF AND EXCLUDES ALL OTHER WARRANTIES, EXPRESS OR IMPLIED, INCLUDING WITHOUT LIMITATION, MERCHANTABILTIY OR FITNESS FOR A PARTICULAR PURPOSE.

Medical Monofilament has made a diligent effort to accurately illustrate and describe the product in its product literature.  However, such illustrations and descriptions are not a warranty.  This warranty does not cover any failure of the monofilaments or any injury arising from misuse, negligence, accidental damage, or tampering with the product.  Medical Monofilament shall not be liable to any party for failure of the monofilaments or any injury resulting from misuse of the monofilaments or accidental damage or tampering to the monofilaments caused by anyone other than Medical Monofilament.  The product should be used as a diagnostic aid only in accordance with standardized instructions, and test results should be shared regularly with a qualified medical professional regardless of the results.  Medical Monofilament shall not be liable for any special, indirect, incidental or consequential damages.

While our Open PO offers customers the ability to order in bulk and obtain the best possible pricing, it is also important given the product’s expiration date. In order to comply with FDA guidelines of guaranteeing the 10-gram force range, Medical Monofilament has placed a one-year expiration date on all monofilaments. Monofilaments are tested for one year.  With incremental shipping, you will receive monofilaments run closer to the ship date so that there is no risk of receiving and/or sending monofilaments that are near expiration or have already expired. We ship to one central location determined by the customer.

Minimum order requirements for Open PO:

  • 250-20 packs
  • 125-40 packs
  • 200 packs- Monofilaments attached to instruction cards

For in-house monofilaments, we require a one-year duration and cannot issue standing orders for any period of time over one year. The PO must include quantity of packs and once the PO is completely fulfilled, a new PO must be issued. Medical Monofilament reserves the right to set terms at prepay, 50% deposit, or Net 30. There is a 10-day turnaround time for the first order.

Shipping and payment options:

Shipping options include but are not limited to weekly, monthly, quarterly, or semi-annually. If a client chooses not to have an assigned ship date, the ship date will be determined by the number of orders ahead. Due to the size of Open POs, accepted shipping could be delayed up to 3 weeks. Quantities to be shipped will be determined by the quantity ordered and agreed upon before open PO is accepted.

25% custom deposit for custom orders – If a customer places a custom order with incremental shipping dates, payment options are prepayment in full or a 25% custom deposit before the work order is issued. The 25% custom deposit will be deducted from the final invoice. Payment and shipping schedule will be pre-determined. If we do not receive payment the ship date will be delayed. If payment is received late, Medical Monofilament reserves the right to deny Open POs. Quantities to be shipped will be determined by quantity ordered.

All open PO’s are for up to one-year’s duration. The first shipment time will be dependent upon the quantity ordered and orders ahead. If there are other orders ahead of your order, start day may be delayed several days. Following shipments will have a guaranteed 10-day turnaround time. For in-house monofilament orders, payment is net 30. The 25% deposit will be deducted from the last shipment for custom orders.

Medical Monofilament is happy to accommodate custom orders. If you are interested in a custom order, please contact us with your name and phone number. One of our customer service representatives will contact you as soon as possible to personally assist you. The name and number of a contact person responsible for making decisions regarding custom orders must be provided. Our minimum order requirement for custom orders is 5000 units.

Medical Monofilament is not responsible or liable for Art or Logos received for custom monofilaments and/or instruction cards that is copywrited, trademarked or registered. You can find more details about our custom orders here.

Custom monofilaments are subject to custom terms. Custom PO must include quantity of packs, a contact name, phone number and email, a complete ship-to address and preference of shipping options. Once a new PO is satisfied, a new PO must be created.

To place an order for Alaska, Hawaii, U.S. territories or other international destinations, please contact us here. One of our customer service representatives will contact you within one business day to personally assist you.

All international orders must be prepaid unless other arrangements are made with Medical Monofilament. Payments must be in U.S. currency. VISA, Master Card, electronic transfers or money orders are accepted. For Canadian customers, checks must be drawn from a U.S. bank. VAT, as well as duties and taxes, are the responsibility of the customer. Credit card information must be provided by phone or fax, as information sent via email is not fully secure.

Orders may be incrementally shipped. Details will be determined once the size of the order is known. If the customer chooses to use their preferred carrier, Medical Monofilament will only be involved by placing the delivery label(s) on the box.

UPS shipping rates apply to the 48 contiguous states. Alaska, Hawaii and U.S. territories are subject to different rates. If the customer chooses to use their preferred carrier, Medical Monofilament will only be involved by placing the delivery label(s) on the box.

Orders placed over the weekend will be processed on the next business day.

Expedited shipping must be ordered before 12pm EST to guarantee same-day order completion. If customers choose expedited shipping options, Medical Monofilament cannot guarantee orders over 100 blister packs of the 20-pack or 40-pack products, or over 50 blister packs of the 25-pack product with monofilaments attached to instruction cards.

In case of inclement weather, shipping time may be delayed.

Online orders require prepayment. Purchase orders payment terms are net 30 or at the discretion of Medical Monofilament, and prepayment may be required. 18% finance charge will be added to overdue invoices. UPS shipping rates apply to the 48 contiguous states, not including Alaska, Hawaii or U.S. territories. Custom orders and international orders (including Alaska, Hawaii and U.S. territories) will have different terms.

We reserve the right to add any administrative or other fees, which may be charged to Medical Monofilament to the amounts due in connection with any orders placed. We offer quantity discounts to our clients.  It is the decision and responsibility of the client whether or not to take advantage of these discounts.

Refund is allowed if product is returned within 30 days of order placement. There is a maximum return of 1,000 items allowed. A return authorization form must be completed, which can be downloaded at this link. Product must be unopened and in good condition. Product must be returned in original package.  If original package is damaged or destroyed, we cannot accept the return.

20% restocking fee may be applied. Customer is responsible for paying return shipping charge unless the return is a result of our error. Custom orders are non-returnable, non-refundable.

Damaged goods must be reported within 24 hours of delivery. Proof of delivery must be made within 30 days of shipping date. If there is a product or packaging recall the customer will be notified immediately. The customer has 7 days after notification to return the product. Medical Monofilament will pay shipping charges back to Medical Monofilament, replace the order and pay shipping charges back to the customer.
